While we will review your request, we are not required to agree to all restrictions.

D. Right to Confidential Communications

You may request alternative communication methods, such as:

  • A different email address

  • Mailing correspondence to an alternate address

We will accommodate reasonable requests.

E. Right to an Accounting of Disclosures

You may request a list of certain disclosures of your PHI made over the past six years.

F. Right to a Copy of This Notice

You may request a paper or electronic copy of this Notice at any time.

  • Comply with all applicable federal and state privacy laws

We reserve the right to modify this Notice at any time. Changes will apply to all PHI we maintain and will be posted on our website with a new Last Updated date.
