Schreiber Concierge & Longevity provides:

  • Metabolic and hormonal support, including medically guided GLP-1 microdosing programs

  • In-home, local, and travel-based consultations

  • Seasonal, long-term, and remote membership medical services

Membership is not health insurance and does not cover:

  • Hospital care

  • Emergency room visits

  • Laboratory fees

  • Imaging services

  • Prescription costs

  • Services rendered by third-party providers

2. No Emergency Services

The Practice does not provide:

  • 911 or ambulance services

  • Emergency room care

  • Care for life-threatening conditions

If you are experiencing an emergency, call 911 immediately.

3. Membership, Billing & Payment

By enrolling in a membership or purchasing any service, you acknowledge and agree that:

  • You authorize the Practice to charge your selected payment method

  • Membership fees may renew automatically per the terms of your agreement

  • Certain fees may become non-refundable once services commence

  • Pricing may vary based on location, travel requirements, and service tier

4. Scheduling, Cancellations & Availability

You agree that the Practice may:

  • Adjust or reschedule appointments when necessary

  • Decline appointments if the requested service is clinically inappropriate

  • Charge cancellation or no-show fees as outlined in your Membership Agreement

You agree to provide accurate and complete information when booking.

5. No Medical Advice Without Evaluation

Communications through:

  • Website forms

  • Email

  • Text messaging

  • Any online or app-based messaging platform

do not constitute medical advice unless you are an established patient who has already been evaluated in person.

New patients must undergo an initial examination before receiving medical guidance, diagnoses, or treatment.

6. SMS Communications

By providing your phone number, you consent to receive text messages from Schreiber Concierge & Longevity regarding your care, scheduling, service updates, and important practice information. Message frequency may vary. Standard message & data rates may apply.

You may opt out of SMS communication at any time by replying STOP.
